HENRY TAYLOR, MPA
President and Chief Executive Officer

Mr. Taylor, a native of the City of Chicago, spent most of his formative years on the south side.  He was formerly educated via the Chicago Public Schools and received his Master's Degree in Public Administration from Roosevelt University.  He has also completed Executive Management training at Wharton (University of Pennsylvania), UCLA, University of Kentucky and Harvard.  He has over twenty years of Health Care Administrational experience in the private, non-for-profit, Catholic and public sector.  He is passionate regarding the patent's rights to health care.  He continues to advocate for the rights of individual patients and actively lobbies for increased access to health care for underserved families on a national and local level.  He currently serves as Chairman of the Board for the Illinois Primary Health Care Association (IPHCA.)  In addition, he has served as past Chair of the National Association for Community Health Centers Task Force on Primary Care for Public Housing, a Member of the Board of Directors for CQuest America Inc., past Board Member for the Chicago Children's Choir and volunteer for the Boy Scouts of America. Mr. Taylor has been with Miles Square since 1998.

 

TAMARAH DUPERVAL, MD
Medical Director

Dr. Duperval is a Family Medicine Physician and an Assistant Professor of Clinical Family Medicine at the University of Illinois at Chicago College of Medicine. As Medical Director, she provides leadership and oversees medical staff and clinical services for several clinic sites, provides guidance for research initiatives, and provides direct delivery of health care from pediatrics to geriatrics, including obstetric care. Her clinical interests are in the area of health care policy for underserved populations - specifically related to access, workforce development, and quality of care initiatives. She is a member of the Board of Directors of the Illinois Academy of Family Physicians. Dr. Duperval earned a Bachelor of Science in Chemical Engineering at Northwestern University and received her Medical Degree from the University of Illinois at Chicago, College of Medicine. She completed a residency in Family Medicine at West Suburban Hospital in Oak Park IL and later completed the Commonwealth Fund/Harvard University Fellowship in Minority Health Policy as well as earning a Master of Public Health Degree at the Harvard School of Public Health in 2000. She participated in the Marshall Memorial Fellowship as a Fellow in 2008.  

 

LESLEE D. SLAUGHTER, DDS
Dental Director

Dr. Slaughter earned her DDS from the University of Illinois at Chicago, where she also completed her Pediatric Dentistry residency program. Some of her past research has focused on access and utilization of dental care services for minority children. She is dedicated to delivering quality dental care to any child who needs it, while trying to make receiving dental care a positive experience for children.

 

RHONDA MUNDHENK, JD, MPH
Associate Executive Director and Chief Operating Officer

Ms. Mundhenk was promoted to Associate Executive Director in 2009, though she has been an employee of Mile Square since 2005. She is responsible for working closely with the Executive Director (CEO), Medical Director, CFO, and senior management team to ensure that patient care and operations run efficiently.  She develops and motivates an administrative team to manage the Health center, is accountable for facilities, ensures policies and procedures for functional areas, and ensures compliance. Ms. Mundhenk holds an MPH from the University of Illinois at Chicago School of Public Health, and a JD from Northwestern University.  Her BA in Anthropology is from Westmont College.  Prior to becoming CCO of MSHC, she was the Director of Development.  Previous positions included Director of Resource Development and In-House Counsel for Roseland Community Hospital.

 

ROBERT TAYLOR, CPA
Associate Director of Financial Affairs and Chief Financial Officer

Mr. Taylor (no relation to Henry Taylor) was hired as CFO during 2009. He reviews, analyzes, interprets and communicates financial reports and data and ensures the integrity of same; maintains a formal budget program; ensures compliance with external financial regulations and standards; participates in long range planning and development; and coordinates external audits. Before joining MSHC, Mr. Taylor was in private practice in financial services.  Earlier, he served as CEO/CFO of Friend Family Health Center in Chicago, and held a number of positions in finance at the University of Chicago.  He is a CPA and holds a BS in Accounting from the University of Illinois.
