Henry Taylor, MPA

President and Chief Executive Officer

Mr. Taylor, a Chicago native, spent most of his formative years on Chicago’s South Side. He was educated via the Chicago Public Schools and received his Master's degree in Public Administration from Roosevelt University. He has also completed Executive Management training at the University of Pennsylvania Wharton School, UCLA, University of Kentucky, and Harvard. He has over 25 years of healthcare administrative experience in the private, nonprofit, Catholic, and public sectors.

Mr. Taylor is passionate about patients' rights to healthcare. He continues to advocate on a national and local level for the rights of individual patients and actively champions increased access to healthcare for underserved families. He is past chair of the Board of the Illinois Primary Health Care Association (IPHCA), past Chair of the National Association for Community Health Centers Task Force on Primary Care for Public Housing, past speaker of the house for the National Association of Community Health Centers, and is currently a board member for many not-for-profit organizations. Mr. Taylor has been the CEO of the Mile Square Health Center since August 1998.

Ian Jasenof, MD, FACOG, MHA, CPE

Chief Medical Officer

Dr. Ian Jasenof was born and raised in Philadelphia. He completed his undergraduate degree at the University of Illinois Champaign-Urbana. He subsequently completed his medical degree at Rush Medical College and completed his residency in Obstetrics & Gynecology at the University of Illinois at Chicago. He has over 25 years of clinical experience.

His healthcare leadership roles include Director of Undergraduate Medical Education, ex-officio member Board of Directors for Advocate Lutheran General Hospital, and various system-wide leadership initiatives, including Quality Improvement, Peer Review, and Credentialing.

Dr. Jasenof has extensive experience in the Federally Qualified Health Care (FQHC) arena, where he has served as a site medical director. He brings these experiences to promote best practices for those communities at risk for poor health outcomes. His servant leadership style as well as an innovative team-based approach to change management has led to improved health outcomes.

Timothy J. Stecker, CPA

Chief Financial Officer

Mr. Timothy Stecker was born and raised in Wisconsin, and after living over 20 years in Denver, Colorado, moved back to the Midwest and joined Mile Square Health Center in March 2018. He completed his undergraduate degree at the University of Wisconsin-Milwaukee and is a licensed CPA in the State of Colorado.

With more than 25 years of healthcare financial leadership experience, Mr. Stecker has worked in multiple facets of the healthcare continuum, ranging from family practice, oncology, and behavioral health to hospitals, skilled nursing facilities, and home health organizations.

Sodabeh (Sue) Etminan, DMD, MPH

Dental Director

Dr. Sodabeh Etminan grew up in Rockford, IL. She completed her undergraduate degree at the University of Illinois Chicago and her dental degree from the University of Pennsylvania. She returned to UIC to complete her Masters in Public Health.

She has over 15 years of clinical experience in the community health setting and has leadership roles or committee appointments in the Illinois Primary Care Association and Illinois Department of Public Health. She also sits on the Advisory Council for the National Health Service Corps. She is involved in training the next generation of public health providers and serves on the advisory council for the Schweitzer Fellowship.

Dr. Etminan was named one of the Top 40 Dentists under 40 by an industry publication and was awarded an Emerging Leader Award from the Health and Medicine Policy Research Group. She strives to provide quality dental treatment settings for all patients with an increased focus on interdisciplinary care and improved outcomes. She also has an interest in research and is the PI on several foundational and federal grants. 

Nicole Gastala, MD

Senior Director of Research, Behavioral Health and Addiction

Dr. Gastala is board-certified in Family Medicine and Addiction Medicine. She graduated from Loyola Stritch School of Medicine in Chicago and completed her Family Medicine residency at the University of Iowa. Dr. Gastala has dedicated her clinical career to caring for patients in Federally Qualified Health Centers and started her work in rural Iowa as a National Health Service Corps member. There, she developed an integrated Primary Care and Medication for Opioid Use Disorder program with her colleagues to address the unmet health needs of the community, the first within an FQHC in the state of Iowa.

In her clinical and leadership role at MSHC, she has developed an integrated behavioral health, addiction, and primary care program within Mile Square with the goal of removing structural barriers to care including walk-in visits and immediate same-day referrals between primary care and behavioral health. She has co-developed one of the first integrated and co-located opioid treatment programs within an FQHC with community partner Family Guidance to ensure patients have access to all FDA-approved medications for opioid use disorder. In her research role, Dr. Gastala leads the research, innovation, service, and evaluation (RISE) division of MSHC that supports the system in service grant development, expansion of patient programs, and assists researchers in the implementation of programs at Mile Square Health Center.

Phyllis Grice MSN, RN

Chief Operating Officer

Ms. Grice, a Chicago native, was raised on the North Side. She is a Registered Nurse, completed her undergraduate degree at Olivet Nazarene University, and received her Master of Science in Nursing from Walden University. Her career started in the emergency department as a staff nurse, moving into the role of leadership. She served in the emergency department leadership at Mt. Sinai, Stroger, and Holy Cross. She is a Trauma Nurse Specialist and was instrumental in developing the Stroke Program for Holy Cross Hospital.

She has over 25 years of clinical experience and currently serves on the operations, clinical, and community development committee for the Illinois Primary Health Care Association (IPHCA). She belongs to a variety of nursing organizations including the Emergency Nurses Association, Society of Trauma Nurses, and the American Nurses Association. She is passionate about bringing holistic quality healthcare into the communities.

Charisse Franklin, MP, CPHQ, LSSGB

Senior Director of Administrative Operations

Charisse Franklin is a Chicago native, raised on the South Side. She received her grammar and high school education in the Chicago Public School system and obtained both her undergraduate and graduate degrees from the University of Illinois at Chicago. Ms. Franklin has a Bachelor of Science degree in Health Information Management, a Master of Public Health degree, and is a Certified Professional in Healthcare Quality (CPHQ).

Ms. Franklin has more than thirty years of management experience, twenty-three years of leadership within UI Health, and four years managing a Federally Qualified Health Center. She has education and skills in team building, needs assessment, organizational management, and conflict resolution. She also has a Green Belt in Lean Six Sigma improvement methodology. Ms. Franklin has assisted in the development, implementation, and management of new and existing clinic locations. Ms. Franklin has received several awards including the UIC College of Nursing Alvie Bailey Award for leadership, commitment, and excellence, the Julie and Mark Zerwic Diversity Award, and the Power of Nursing Leadership Pinnacle Leader Tribute for being an extraordinary leader.

In this role, Ms. Franklin oversees business activities to ensure Mile Square strategies and goals related to business development, research, quality improvement, community and stakeholder engagement, and compliance are achieved. Ms. Franklin is passionate about serving the underserved and working to eliminate the gap in health equity because health and quality healthcare are fundamental human rights.

Kimary Lee, JD, MUPP

Senior Director Compliance & Development

Kimary Lee joined Mile Square Health Center at the University of Illinois Hospital & Health Sciences System in February 2012 as Director of Compliance and Development. In May 2019, she was promoted to Senior Director of Compliance and Development. In her compliance role, Ms. Lee is responsible for establishing and implementing the health center’s compliance program across the Mile Square Health Center enterprise, including review and evaluation of regulatory compliance matters impacting the health center; internal investigations; and monitoring and reporting.

Ms. Lee’s additional responsibilities include management of the health center’s federal Section 330 Health Center Program grant portfolio and ensuring compliance with federal Health Center Program requirements. Ms. Lee serves as co-chair of the health center’s Risk Management Committee and its Policy and Procedure Committee. She is also a member of the Risk Management Committee of the Illinois Primary Health Care Association.

Ms. Lee has a Juris Doctor degree from Georgetown University Law Center in Washington, D.C., and a Master of Urban Planning and Policy degree from the University of Illinois at Chicago.

Ms. Lee is a native Chicagoan, having been raised on Chicago’s West Side and formally educated in the Chicago Public School system.

Nathan Stackhouse, MD, MPH

Associate Chief Medical Officer

Dr. Nathan Stackhouse was born and raised in Colorado but has spent the last 20+ years of his life in the Chicagoland area. He completed his undergraduate education at Northwestern University, followed by his medical degree and residency at the University of Illinois at Chicago (UIC). Dr. Stackhouse subsequently completed a one-year Academic Career Fellowship with the Department of Family and Community Medicine at UIC. He obtained his Master of Public Health at UIC in 2017.

His interests include patient-centered medicine, health equity, quality improvement, healthcare administration, and medical education.

Angela Ellison, M.Ed

Senior Director of School Based Practice

Angela Ellison is currently the Senior Director of School Based practice for Mile Square Health Center. In this role, Ms. Ellison oversees contract compliance and provides leadership to five school-based health centers and an HRSA-funded Healthy Start, maternal and child program in partnership with Mile Square. Ms. Ellison utilizes her skills to raise for money the operation of school-based health centers while also ensuring the program meets all federal, state, and university policies and guidelines.

Ms. Ellison is a native-born Chicagoan who has a lifelong commitment to addressing the health and wellness of families in the Chicago land area with a specific focus on marginalized communities. School-based health centers are full-service health centers able to meet the health and wellness needs of the entire family. These school health centers are designed to address barriers to care such as transportation and missing work for the parents and students in order to minimize expenses yet facilitate access to quality healthcare in their neighborhood/schools.

Ms. Ellison has a master’s degree in education and over 35 years in leadership and administrative roles. She is a lifelong learner and takes on the leadership role, when necessary, to use as an opportunity to help improve lives.

Ms. Ellison is an advocate for positive change and is committed to improving the quality of life for the families and individuals she serves.

Tareg Qtami, MPA

Director of Human Resources

Mr. Tareg Qtami is the Director of Human Resources. He is responsible for overseeing all human resources activities, including the administration of recruitment, pre/post-hire transactions, employee relations, employee engagement, HR compliance, and payroll functions across the Mile Square Enterprise. In addition, Mr. Qtami leads and develops staffing strategies and implementation plans to identify talent within and outside of the Health System. As a strategic partner, Mr. Qtami aligns Mile Square objectives with employees and management through coordinated recruitment & retention efforts, including the implementation and evaluation of recruitment strategies throughout the Mile Square Enterprise and Community Based Health.

Mr. Qtami joined Mile Square as Director of Human Resources in 2023. Prior to joining Mile Square, he started his human resource career at UI Health in 2012. During his career at UI Health, he has held numerous roles within Human Resources.
He received his Master’s Degree in Public Administration from the University of Illinois at Chicago and has a Bachelor of Fine Arts degree from the Illinois Institute of Art - Chicago.

At age seven, Tareg immigrated to Chicago, Illinois from Jordan. He was raised in Chicago and formally educated in the Chicago Public School system.
