We hope to see you and your family regularly, so that we can work together to help promote healthy lifestyles and provide you with high-quality resources.

Please call 312-996-2000 to make an appointment.

Please arrive 15 minutes before your appointment to register. If you are a new patient, please arrive 30 minutes before your scheduled appointment time.

Should you need to cancel, please let us know at least 24 hours in advance.

Same-day appointments are available on a limited, first-come-first-served basis, and wait times will vary. Our Urgent Care Center also is available – however, due to demand, there may be times that the Center is no longer accepting new patients.

Mile Square Health Center provides medical care to everyone, regardless of their ability to pay. Mile Square accepts a broad range of payment and insurance plans. Based on their income and family size, many of our patients qualify for low-cost services.

When you come to Mile Square, with an appointment or for convenience care, please bring the following:

  • An insurance card for yourself and anyone else in your family who is receiving care (if you have one)
  • A list of all medications you currently are taking, including vitamins and dietary supplements
  • If your children are receiving care, please bring their immunization cards

If you are a regular Mile Square patient with a serious but non-emergency medical need when Mile Square clinics are closed, please call 312-996-2000 and ask for the physician on call.
