Who is this program designed for?
The Self-pay program is for qualifying uninsured and/ or underinsured patients as determined by Mile Square Health Center . The pharmacy is able to assist the self-pay patients in obtaining medications at a sliding fee scale.
How do they sign up for it, or use it?
Patients must be a registered patient with Mile Square Health Center , Mile Square Urgent Care or any of the Mile Square clinics located throughout the city. To utilize the sliding fee scale for medications the patients need to have their Mile Square provider send their prescriptions to the Mile Square Pharmacy.
Are there any forms or papers they need to fill out?
There is registration paperwork that Mile Square Health Center will require the patient to complete and also bring in the appropriate documentation to establish proof of income.
