Below are the print ready PDF files used by different providers and clinics within UI Health. To order a reprint of these files please contact UIC Publications at publications@uic.edu with the following information: FOPAL for payment, quantity, delivery address with mail code, and name of the file. Once UIC Publications has your information, they will respond with pricing and delivery schedules within 2 business days.
