Our Diabetes Prevention Program provides personalized care and resources to reduce your risk of developing diabetes and manage prediabetes effectively. Through comprehensive evaluations, tailored education, and support from our dedicated team, you can make the lifestyle changes necessary to live a healthier life.
Am I at Risk for Diabetes?
Diabetes risk factors can vary, but some common indicators include:
- Family history of diabetes
- Being overweight or obese
- Lack of regular physical activity
- High blood pressure or cholesterol
- A history of gestational diabetes or giving birth to a baby over 9 pounds
- Being over 45 years old
- Belonging to certain racial or ethnic groups (African American, Hispanic/Latino, Native American, or Asian American)
Our team can help assess your risk and provide actionable steps to prevent diabetes.

Our program offers a variety of services designed to empower you to take control of your health:
- Personalized lifestyle coaching
- Nutrition counseling with registered dietitians
- Group education sessions to support behavior changes

Conditions We Treat
Our specialists provide care for the following:
- Prediabetes
- Insulin resistance
- Metabolic syndrome
- High blood pressure
- Obesity or overweight
If you’ve been diagnosed with any of these conditions or have risk factors, we’re here to help you create a personalized prevention plan.
Where the Program Takes Place
Our Diabetes Prevention Program is offered in a convenient hybrid format, with both in-person and virtual options available:
- Current Meeting Location: Primary and Immediate Care Center in Auburn Gresham
- Virtual Option: Sessions available via Zoom
Coming Soon: Sessions will also be offered at the Davis Health & Wellness Center, with hybrid options for both English and Spanish-speaking participants.
