UI Health’s 55th & Pulaski Health Collaborative provides comprehensive healthcare services for the residents of Chicago's West Elsdon and Gage Park communities on the city’s Southwest Side.

By increasing access to services like urgent care, behavioral health, dental care, women's health, and advanced diagnostics, 55th & Pulaski is committed to providing high-quality care alongside educational, financial, and social support to guide patients through their care journey.

To understand how this care is impacting our patients and their community, we spoke to the associate dean of clinical affairs for UIC’s College of Medicine, Dr. Heather Prendergast, and an on-site wound care specialist, Dr. Jobin John.

What makes 55th & Pulaski such a unique clinic?

The 55th & Pulaski Health Collaborative takes a different approach to care. Instead of treating specific symptoms or conditions, providers look at the whole person, including the needs — social, emotional, financial, etc. — that impact them beyond the four walls of the clinic.

Of course, this care includes access to highly specialized diagnostics and treatment options, but it also includes on-site community programming, such as healthy cooking classes and mental health information sessions and considers the importance of staff who live in the same communities as our patients.

“Previously, patients would have had to travel to our downtown campus to access to these comprehensive services under a single roof,” said associate dean of clinical affairs for UIC’s College of Medicine. “This model of convenient, accessible, and specialized care is why we’re proud of the services and impact our clinic has had.”

What specialized care services are available at 55th & Pulaski?

Services at 55th & Pulaski include more than 20 adult specialties, including Cardiology, Endocrinology (Diabetes), and Nephrology (Kidney Disease); Occupational & Physical Therapy, Pediatric Specialty Care; Radiology services; and Women’s Health. Patients can get treated for chronic and acute conditions that require advanced testing, diagnostics, and care from a highly trained provider.

Providers are also able to help patients manage chronic conditions like diabetes and high blood pressure that may impact the specialty care they’re seeking.

“I practice down the hall from our endocrinologists and cardiologists, which is a blessing when I’m treating a patient with diabetes or high blood pressure,” said , a wound care specialist at 55th & Pulaski. “I can immediately consult with one of my colleagues, and together, we’re able to put together a plan that accounts for all a patient’s health needs.”
