UI Health is offering the Pfizer COVID-19 vaccine to those 5 years or older at the UI Health Pilsen Family Health Center at no cost during two special Saturday clinic.
SATURDAY JANUARY 22, 9 AM TO 2 PM
- First dose COVID-19 vaccine for those ages 5+
Your second dose appointment will be scheduled for Feb. 12 - COVID-19 booster for those ages 12+ who have been fully vaccinated for 6 months
- Flu Vaccine
SATURDAY FEBRUARY 12, 9 AM TO 2 PM

- No proof of insurance or ID needed

Need a consent form from a parent or legal guardian to
