PRE-ME Health Assessment Tool

Determine Your Delivery Plan on Your First Visit

UI Health's new partnership with March of Dimes allows us to establish your care plan on your first visit. With our new PRE-ME Health Assessment, UI Health physicians will review your health history and conclude the level of care you may need to deliver a safe and healthy baby.

When you first visit one of UI Health's locations — Family Medicine Center at University Village, Outpatient Care Center, or one of our Mile Square Health Centers — our physicians will take the time to record your personal health history and complete our PRE-ME Assessment. By completing the assessment, UI Health will provide you and your Labor and Delivery team with a risk evaluation and a care plan during your first visit. You and your provider will reassess your plan at 18 and 24 weeks of pregnancy.

If our PRE-ME assessment determines you to have low risk, you will be provided with a care plan and a Labor and Delivery Team specially trained to help guide you through to your delivery day.

If our PRE-ME assessment determines you to have high risk, the UI Health Labor and Delivery team, composed of physicians, advanced practice nurses, and community and social workers, will provide you with the resources you will need during your pregnancy.
