Proof of Vaccine Status with MyChart
Many public establishments require proof of COVID-19 vaccination for entry. You can use MyChart on a desktop computer to download your COVID-19 Vaccination Record or use the MyChart App’s COVID-19 QR Code Verification on your smartphone for proof of vaccination at participating locations.
Create a COVID-19 QR Code Verification in MyChart to scan at participating locations.
- After opening MyChart on your smartphone or device, click on the “QR COVID” icon at the top left of the screen.
- At the bottom of the COVID-19 screen, click on “QR Codes.”
- The QR Code can be read by a QR Code scanner and provides your vaccine status at participating locations.
