To refer a patient, please follow one of the options below:

For seamless referrals and to ensure continuity of care, please consider utilizing Epic or EpicCare Link.
- Fax Documents
Please fax a copy of your order and the patient's demographic information/factsheet to 312.355.5838.
If the patient has complex contraceptive needs, please include any relevant clinic notes or procedure notes of attempted device insertions/removals. If the referral is related to pregnancy, please send all of the following (when applicable):- Ultrasound reports
- Prenatal records
- Fetal genetic screening or testing results
