University Health Service

University Health Service (UHS) provides occupational health services to employees of the University of Illinois Hospital & Clinics (UI Hospital), University of Illinois Chicago, UI Hospital volunteers, and health sciences students.
Our mission is to promote the health of the employee, volunteer, or student throughout their work at the university.

For Employees Injured on the Job

A university employee who is injured on the job must inform their supervisor promptly to report the injury. An employee and their supervisor must thoroughly complete and sign the First Report of Injury Form (.pdf) within 24 hours and email (WorkComp@uillinois.edu & Safe@uic.edu) to the Office of Workers' Compensation and Claims Management.
