UIC employees who do not work in the hospital will complete the Immuware respiratory questionnaire and are referred to the Environmental Health & Safety Office to arrange fit testing.
N-95 Voluntary Form
Fit Testing Attestation Form for those that VOLUNTARILY choose to wear a N-95 Respirator, but are not required to wear one based upon their job duties. Complete this form and provide a copy to your Director and the Environmental Health & Safety Office.
