Our Referral Process
After the provider submits their online referral request, the provider can expect a call back from the Customer Care Center within 1 - 2 business days. The patient will be contacted as well if indicated on the initial request.
Information Required When Making a Referral
- Referring Physician Information (UPIN/NPI, provider name, clinic/practice, phone and fax number, address, contact name)
- Patient's Information (name, date of birth, gender, address, phone number, insurance information) --- For pediatric referrals, guarantor information will be needed.
- Reason for the referral, symptoms and diagnosis (include services that have been provided as part of the patient work up)
- Any special needs that will require additional facilitation once the patient arrives for their appointment
