Patient Information for Mile Square Health Centers
At Mile Square Health Center, we provide healthcare services for patients at a variety of life stages, from childhood through adulthood. Our goal is to provide the highest quality healthcare for you and your entire family.
Much of what we do at Mile Square is primary care, or continuous, essential day-to-day healthcare for our patients. We treat acute conditions, such as broken bones and asthma attacks, in addition to chronic conditions, such as diabetes and heart disease. Our healthcare services address our patients’ physical, mental, and social health needs, as well as provide appropriate referrals when necessary.
Mile Square clinics are located conveniently throughout Chicago and in Cicero and Rockford. To accommodate the variety of communities we serve, our staff provides assistance in English or Spanish. Translators are also available for other languages.

How Can I Make an Appointment?
To make an appointment, please call 312.996.2000.
Please arrive 15 minutes before your appointment to register. If you are a new patient, please arrive 30 minutes before your scheduled appointment time.
Should you need to cancel, please let us know at least 24 hours in advance.
Can I Walk In and See a Provider?
Same-day appointments are available on a limited, first-come first-served basis, and wait times will vary. Our Urgent Care Center also is available. However, due to demand, there may be times when we are no longer accepting new patients.
How Much Will I Pay?
Everyone deserves high-quality healthcare. At Mile Square Health Center, we provide medical care to everyone, regardless of their ability to pay. We also accept a broad range of payment and insurance plans. Based on income and family size, many of our patients qualify for low-cost services.
What Should I Bring?
When you come to Mile Square for an appointment or convenience care, please bring the following:
- An insurance card for yourself and anyone else in your family who is receiving care (if you have one)
- A list of all medications you currently are taking, including vitamins and dietary supplements
- Your children’s immunization cards (if they have them and if they are receiving care)
Are Language Services or Hearing Resources Available?
Yes. We are committed to inclusive care, including language services to patients who are hearing impaired and/or their companions whose preferred language is not English. To ensure effective communication regarding patient care, medical language interpreters, sign language interpreters, and transparent masks are available upon request. Contact Interpreter Services at 312.413.9620.
What If I Need Care After Hours Or I Have An Emergency?
If you have an emergency at any time, please go to the emergency room at the University of Illinois Hospital & Health Sciences System located at 1740 W. Taylor St., Chicago, IL 60612, 312.355.4000, or the hospital nearest you.
If you are a regular Mile Square patient with a serious but non-emergency medical need when our clinics are closed, please call 312.996.2000 and ask for the physician on call.
