Family Medicine
The Department of Family Medicine at the University Of Illinois Hospital & Health Sciences System provides high quality, comprehensive healthcare for people of all ages - infants through adulthood. Our physicians and nurse practitioners are committed to providing culturally sensitive medical care, health maintenance and preventive services.
We provide comprehensive and continuous care to each member of your family regardless of gender, age or type of problem. As a leading academic medical center, our medical students and resident doctors have extensive contact with our patients. We believe the collaborative approach, patients working with us to decide on the best type of care based on individual needs, is the best approach.

Medical Home
The Family Medicine Center is committed to providing high quality care for all of your needs. We are here to provide you with a "medical home," taking the worry out of navigating through a complex system and doing it for you. The Department of Family Medicine is a direct referral connection to the many exceptional specialty care practice areas, which have received national recognition for excellence. When you choose us as your medical home you will receive convenient, confidential, accessible, and affordable care all at one place - UI Hospital.
We recommend regular physical exams for your optimal health and well-being.
Appointments
The Family Medicine Center operates by appointment only. Appointments can be scheduled in person or by phone at 312.996.2901. When you schedule an appointment, we'll need some basic information:
1. Your name, current address, and telephone numbers (two preferred).
2. You date of birth.
3. Health Insurance Information
4. Your primary care physician or nurse practitioners name.
5. Specific information about why you need to see a clinician so we can schedule an appropriate amount of time.
Hours
For your convenience the Family Medicine Center at University Village offers extended hours. To accommodate your busy lifestyle, the clinic sees patients Tuesday and Thursday evenings until 7:00 p.m. Additionally, urgent care appointments are available Saturday mornings from 8:30 a.m. - 11:30 a.m.
Monday, Friday 8:30 a.m. - 5: p.m.
Wednesday 9:00 a.m. - 5:00 p.m.
Tuesday, Thursday 8:30 a.m. - 7:00 p.m. University Village
Saturday 8:30 a.m. - 11:30 a.m. University Village
Students
Many preventative outpatient services are available to all students enrolled in class at UIC. When a student chooses CampusCare as their healthcare plan, Family Medicine providers offer comprehensive primary care services, as well as manage any necessary specialty care. Students who waive out of CampusCare should consult their health insurance policies to determine how to best manage their health care needs. Most insurance plans are recognized at the Family Medicine Center where we can offer you a "medical home" for complete primary needs. All students must present their student ID cards.
Mile Square
True community oriented primary care is provided at the Miles Square locations. Serving as Federally Qualified Health Centers, Mile Square serves Chicago's Near West Side, which includes many families with young children, as well as a substantial geriatric population with multiple chronic health needs. Our Team
All our family physicians are board-certified and are active faculty members of the UIC College of Medicine. They are dedicated to the advancement of medical knowledge through education and research in addition to superior service to their patients.
