To refer a patient or request a consultation:

Please call the Referral Center during regular business hours at 888.842.1801 or fillout our online Physician Referral Form.

Our Referral Center will connect you with experienced representatives who can assist you and your patient. They will expedite appointments for your patient and confirm all details in writing, if requested. Our goal is to make your referrals to UI Hospital as convenient as possible for you and your patient.

To transfer a patient:

Please call our transfer center at 312.996.6226. The Transfer Center is available 24 hours a day, seven days a week and coordinates the smooth transfer of patients from outside facilities to UI Hospital.

Medical Toxicology is a medical sub-specialty focusing on the diagnosis, management, and prevention of poisoning and other adverse health effects due to medications, occupational and environmental toxins, and biological agents.

UI Hospital's Medical Toxicology Service is a partner with the Illinois Poison Center. We offer adult and pediatric inpatient and emergency department consultations.
