Medicare Drug Benefit Consults
The Medicare Drug Benefit Consult Service educates eligible Medicare beneficiaries about the available Prescription Drug Programs and Medicare Advantage Prescription Drugs.
Medication Assistance Program (MAP)
The Medication Assistance Program (MAP) attempts to find long-term solutions for patients who have limited incomes or financial resources and who take chronic medications.
Medication Therapy Management Clinic (MTMC)
Our mission is to assist patients who have multiple chronic medications with the management of their drug therapy in order to improve or maintain their health and prevent or minimize drug therapy-related problems.
Rx Bag & Go
This program is offered by the Wood Street/PCC Pharmacy and offers the convenience of coordinated refills to patients or caregivers of patients. Prescriptions can be picked up at the same time every month.
Smoking Cessation Clinic
This clinic offers patients a consultation to help them develop a personalized cessation plan that uses methods most beneficial to the individual based upon their history and triggers in order to prevent relapse.
Transplant Pharmacy Services
In order to better serve our transplant patients, additional services such as mail delivery, individualized payment plans and pill boxes are offered to patients at no additional charge.
Wellness Center Pharmacy Managed Clinics
The Antithrombosis Clinic assists with management and monitoring of patients who receive anticoagulants and antithrombotics. The service focuses on maximizing the benefits, reducing complications and adverse effects, improving patient outcomes, and decreasing overall healthcare costs of providing antithrombotic therapy. The Hyperlipidemia Clinic program provides services to patients in need of medication and/or dietary management.
