Managing High Blood Pressure (Hypertension)

We are committed to developing a personalized hypertension management plan tailored to your specific needs. We provide individualized care for patients who have high blood pressure along with other complex health conditions, including chronic kidney disease, diabetes, heart disease or stroke risk, hormonal causes of high blood pressure, and high-risk pregnancies.

Our experienced specialists work collaboratively to address kidney-related hypertension and its impact on overall health. Patient referrals to Nephrology can be for various reasons, including difficult-to-control hypertension, and understanding the connection between kidney function and elevated blood pressure.

Ambulatory Blood Pressure Monitoring

Sometimes, patients may be asked to wear a compact, portable device known as an Ambulatory Blood Pressure Monitor (ABPM) to record their blood pressure readings over 24 hours. ABPM is used to help determine the cause(s) of hypertension and related cardiovascular conditions, identify white coat hypertension, uncover masked hypertension, and assess the effectiveness of blood pressure medications during both daytime and nighttime periods.

The monitor includes a blood pressure cuff attached to a small recording unit that can be worn on a belt or shoulder strap. The device automatically captures blood pressure measurements at scheduled intervals—typically twice each hour during the day and once per hour at night.

Approach to Tailored Patient Care

Our collaborative care team focuses on evaluating and managing secondary hypertension, which refers to elevated blood pressure connected to an identifiable cause, such as renal artery obstruction, specific hormonal imbalances, genetic conditions, or tumors. Following a comprehensive assessment, our team partners closely with your primary care physician to deliver a customized blood pressure management approach tailored to your needs.
