How to schedule an appointment

To schedule an appointment for one of our providers you need first to register in the hospital system by calling 312.996.1682 then you can contact our scheduling phone number at 1.800.842.1002

What should I bring with me on my first appointment?

At the time of your first appointment, you should bring your medical records, referring physician's information (name and phone number), appropriate referral, present medication summary, your insurance card, picture ID and a prepared list of questions that you would like to address. Please also bring a food log, exercise log, and patient history form (provided).

Where should I go?

The Nutrition and Wellness Center is located within the Outpatient Care Center located at 1801 W. Taylor Street (Chicago) on the 1st floor, Suite 1C. If you need additional directions, please call our office 312.413.3631  or toll-free 866.600.CARE (2273). 

When should I get there?

Please arrive at least 20 minutes prior to your scheduled appointment. After reporting to the patient care area to register, you will then be seated in a waiting area until you are escorted to an examination/treatment room for your consultation.

How long will I be there?

Expect to spend 1 hour for your first visit; this time is to assure that each new patient is fully evaluated. The length of subsequent visits will depend on the treatment plan and follow-up testing that will be recommended for you. Since there may be a wait before you see the physician please bring something to read or items of interest to occupy your wait time. You may also bring water or a healthy snack. In the main hospital there is a food court for all your food needs. If you or your family member chooses to leave the waiting area, be sure to notify the registration staff.

What will happen while I'm there?

During your visit, the specialist will review your materials and ask questions to help them develop a plan of treatment. The treatment options are discussed and further recommendations are made. Additional testing and referrals may be made based on your needs and plan of treatment. These, as well as follow up appointments will be made at the conclusion of your appointment.

Where should I park?

You may park in the parking structure located at the corner of Taylor Street and Wood Street or Taylor Street and Paulina. Bring the parking ticket with you to have it validated for a discounted rate.
